England Hockey requires all affiliated clubs and organisations to report injuries in line with its Injury Reporting Policy. This is not just a procedural task it is essential for influencing positive change in the sport. Data gathered is reviewed by England Hockey and shared with the International Hockey Federation (FIH), contributing to potential rule changes, equipment reviews, and safety guidance.

England Hockey requires all clubs to complete the England Hockey online ‘Injury Monitoring Form’ or to use the SportSmart reporting app following injuries sustained that require one or more of the following:

• who is removed from the field of play as a result of injury
• who reports an injury immediately after the session (after changing etc)
• who reports an injury, sustained in hockey, that prevents them taking part in other
activities (i.e. if a player contacts the coach or club captain to tell them a niggle/injury has now stopped them doing other sport or activity)
• who reports an injury, sustained in hockey, that required some form of medical
intervention from a practitioner

The reporting of injuries sustained on the field of play is a requirement for all clubs/ organisations affiliated to England Hockey as part of England Hockey's Injury Reporting Policy. The link to the reporting form on the England Hockey website is here and will need to be completed by captains or coaches after each incident which meets the criteria listed - or reported on the app as we change to this system during season 25-26.
